Isabel, nicknamed Belly, grew up with Conrad and Jeremiah Fisher. Their mother, Laurel, is best friends with the boys’ mother, Susannah, so Belly, her mother, and brother Steven have spent every summer at the Fishers’ beach house in Cousin Beach for the last 15 years. Belly has always loved Conrad and has always been friends with Jeremiah, but this year she turns 16 and both boys are beginning to see her differently.
Belly has blossomed into a pretty young woman, but it’s not just her looks that have changed at Cousins Beach. Conrad looks moodier than usual, and Belly learns that he’s recently quit soccer, broken up with his girlfriend, and started smoking. Since Belly still loved Conrad, she could hardly imagine him acting so closed off.
Belly remembers when Conrad broke her heart for the first time. That summer she was 12 years old. Conrad takes her to the local boardwalk where she was hoping for a date but turned out to be an excuse for him to visit a girl he has a crush on.
Susannah, who is like a second mother to Belly, is very worried about Conrad. Susannah’s health is very poor because she has undergone breast cancer treatment and chemotherapy. Belly recalls that she and Jeremiah got together the summer she was 13 when she and Susannah were talking to Laurel about their cancer treatments and crying about the possibility of a mastectomy. Since then, Belly has become a friend Jeremiah can count on in times of family tragedy.
However, Belly’s 14th summer threatens to ruin her friendship with Jeremiah. Belly took her best friend Taylor to Cousins Beach and Taylor immediately started flirting with both Fisher boys. Taylor decides to try and get Conrad’s heart, but Conrad doesn’t get her attention. Jeremiah was, so she flirted with Jeremiah a lot.
In a game of truth or dare, Jeremiah kissed Belly to make Taylor jealous. It hurt Belly’s feelings because it was her first kiss and she wanted to share this moment with someone she really liked, not her best friend.
Right now, Jeremiah is teaching Belly to sled because she’s turning 16 and getting her driver’s license. Steven leaves the beach house to go on a college trip with his dad, leaving Belly without her brother to act as a buffer between her and the Fisher boys. Conrad and Jeremiah become more protective of Belly as her new and improved looks draw the attention of the other boys at Cousins Beach.
Clay, one of her neighbors, invites her to his Fourth of July bonfire. There she meets Cam, a pure vegetarian – he does not drink alcohol, drugs, tobacco, meat products or animal products. Cam likes her. Belly tells Conrad that she is leaving the party and is driving home with Cam in the car.
Conrad forbade her to go. Belly felt that Conrad was treating her like a child and was so angry that she almost cried at the unfairness of the situation. Later, Jeremiah asks Belly to forgive Conrad for his bossy behavior as he is going through a difficult time emotionally.
Belly notices that Susannah sleeps a lot and doesn’t socialize with the rest of the family as usual. Since Mr. Fisher hadn’t visited the beach house once, Belly figured their marriage might be in trouble. A pending divorce would explain Susannah’s reclusive life and Conrad’s mood swings.
Cam invites Belly to a friend’s party. Conrad and Jeremiah insist on joining the party so they can protect Belly. Jeremiah sings a love song on the karaoke machine and dedicates it to Belly and Cam’s new summer romance, embarrassing Belly in front of everyone at the party.
Later, the evening is ruined when Conrad gets drunk and tries to argue with an older man. Belly and Jeremiah try to get Conrad away from the other guy before things get violent, unsatisfactorily ending Belly’s date with Cam. As Conrad and Belly waited in the car for Jeremiah to drive them home, Conrad stroked Belly’s hair. For Belly, the moment of close contact brings back all of her old feelings for Conrad and she realizes that she still loves him.
Conrad tells Belly his parents are getting a divorce, but his mother doesn’t want to admit it to the kids because she wants them all to have one last perfect summer at the beach house. As July turns into August, Belly continues to see Cam and go on dates with him. Laurel and Susannah invite Cam to dinner with the whole family, but it’s awkward because Conrad doesn’t come down for dinner and Jeremiah spends the meal teasing Cam’s jokes.
One night, Conrad came home drunk and playing loud music. Belly wonders if the sudden change in his personality will eventually cause her to stop loving him. But every time Belly Cam hits, her thoughts turn to Conrad. Cam realizes that Belly’s obsession with Conrad is still ongoing, so he and Belly decide not to contact each other after the summer.
One night while the family was watching a movie together, Susannah fell asleep and Jeremiah had to carry her upstairs. When the film ends, Jeremiah and Belly are left alone and Jeremiah confesses that he has feelings for her. He says he hopes she gets over Conrad, but she tells him she still cares about his brother.
Jeremiah’s confession gives Belly the strength to find Conrad and admit that she still loves him. He says he’s not the one for her, but she insists she knows he likes her too. Belly is angry that Conrad is tying her up, unwilling to return her love but still wanting to benefit from her admiration. They had a verbal argument on the beach and Belly cried when Jeremiah pulled up.
Jeremiah yelled at Conrad for making Belly cry, and the brothers argued over the stress built up from an unhappy summer. Laurel said goodbye to her fight.
When Susannah suddenly looks weak, Belly realizes the boys aren’t just happy because their parents are getting divorced or because they both have feelings for them. Conrad and Jeremiah learned that their mother’s cancer had returned. Laurel told Belly that Susannah’s cancer has now spread to her liver, meaning she likely has less than a year to live.
The next day, Belly lay in bed crying and grieving for Susannah’s condition. She ended up having a candid conversation with Susannah, who asked Belly to take care of Conrad in the future. Belly finds Conrad sitting on the beach looking heartbroken at his mother’s fate. She sat with him while he cried. Belly kissed Conrad to comfort him and he kissed her back. He points out that he’s not really thinking about love and relationship issues at the moment, so Belly sits with him on the beach and holds his hand in silent support.
Summer is coming to an end. Belly and Laurel began cleaning up the beach house and packing to go home. On the last night everyone was sitting around the dining table and Susannah talked about her illness and said she wanted to live life to the fullest even though she didn’t have much time left. After dinner, Belly invites Jeremiah and Conrad to join her for a midnight swim in the pool. The three bounced around and had fun together as usual, although it was clear that Conrad and Belly’s relationship had turned romantic.
Months later, during the Christmas season, Conrad drove more than five hours to visit Belly in Boston. He arrived in the middle of the night and she went out to see him. Satisfied with the last Conrad, Belly feels like every summer of her life has brought her to this moment.
